Admissions - Medical Information
Dear Parent,
Forest Lake Academy provides the services of a registered nurse to review each student’s medical records to ensure compliance mandated by the state of Florida and to provide adequate medical care to all students. Upon receiving your acceptance letter, please proceed to complete and return the following mandatory two items to the Admissions office by August 1, 2010:
- Physical examination form - Required for New Students, Dorm Students and Varsity students*
- Immunization Blue form - Required for New Students
*Varsity students must complete their physical examination after June 1, 2010.
Physical evaluation requirements
All new students must have a school physical evaluation prior to the beginning of the school year. It must be dated within the past one year from the first day of school (August 9, 2010). All Residence Hall students must have a school physical prior to the beginning of each school year. All students (NEW and/or RETURNING) trying out for Varsity (Interscholastic Sports Program) must have a physical evaluation prior to each school year.This evaluation must be submitted on a Florida High School Athletic Association Participation Physical Evaluation form and completed after June 1, 2010. (Both sides of the Physical Form)
Health Insurance Policy
All International students must be covered by a Health Insurance policy for the 2010-2011 school year. Proof of Health Care Insurance must be on file with Forest Lake Academy. We strongly recommend that all dorm students are covered by a Health Insurance policy.
Immunization requirements
All new students must submit proof of immunizations. It is mandatory by the State of Florida (statue 1003.22) that no student be allowed to attend any public or private school without proper documentation of the following:
- Immunization form; or
- Exemption of Immunization form as required.
