Medical Services
At Forest Lake Academy, student health and safety are a top priority. Our on-campus clinic provides professional nursing care, first aid, and health guidance in a secure and supportive setting. With a focus on whole-person wellness, rooted in our Christian values, we partner with families to ensure students are cared for and ready to succeed in both their academics and spiritual growth.
REPORTING A SICK STUDENT
Please keep your child home if you feel that they are sick. They must stay home if they have a fever, vomiting, diarrhea, Covid, or the flu. Please call the nurse and let us know that you are keeping your child home or use the Absence/Tardy Form on the FLA website. (There is a quick link button at the top right of the website listed as "Attendance".) If your child has a fever, they must stay home for 24 hours and may not return to school unless they are fever free without medication. They will be excused. MEDICATIONS
Daily Medications: If your child takes a daily medication during the school day, please bring that to the clinic and we will keep it for them to take. Example: a daily ADHD medication at lunchtime. You may also send some extra medication for the clinic in case your child forgets to take their morning medication at home before coming to school. Medications must be in the original bottle with the prescription dosage and name of student.
FLORIDA SCHOOL ENTRY PHYSICAL
In Florida, students entering a Florida school for the first time must have a school entry physical examination completed within 12 months before enrollment. The recommended form is the Florida School Entry Health Exam Form DH 3040.
Key requirements include:
- The physical must be completed and signed by a licensed healthcare provider authorized to perform physical exams in the U.S.
- The exam should include a general health assessment (vision, hearing, height/weight, blood pressure, body systems review, etc.).
- A comparable physical form from another state may be accepted if it contains the same information and was completed within the last year.
- The form must be readable, signed, dated, and usually stamped with the provider’s office information.
- Students must also provide a Florida immunization form (DH 680) or approved exemption documentation.
ATHLETIC FORMS
ECG Requirement: ECGs are required for students planning to try out and/or play on the varsity sports teams. An ECG Clearance Form must also be included when turning in the ECG, giving the student clearance to participate in sports, and needs to be completed before tryouts.
