Transcript Request

Please Note: Forest Lake Academy is not currently set up to process transcript requests through Parchment, even though our school is listed on the platform. Please do not order your transcript through Parchment at this time.

 

Please complete the form below. No fee is needed for current FLA students. Graduates are allowed one (1) transcript at no charge following graduation. There is a $7.50 fee for subsequent transcipt requests. You will receive a call shortly for payment. Allow for 2-3 business days for processing your request after payment has been received. 

You may also mail, fax, or email a transcript request by using this PDF Transcript Request. Requests that are mailed to FLA must include payment by money order or check. Personal checks must clear our bank before the transcript is released. Requests emailed (Email PDF Here) or faxed (FLA Fax: 407-862-7050) should include credit card information for payment.
